Spring 2011 Registration Form

- Players must be registered by March 15th, 2011
- No registrations will be accepted after the drafts!
- $75.00 payment must be submitted at the time of registration.
- Players must move up to their respective age division for 2011-2012 [MAHA Rule]
- League consists of 12 games & 3 practices - completed by May 20th (approx.)

Players with last name beginning with A-L will skate the 1st hour, while M-Z will
utilize the second hour. The final draft will take place after the second hour.
Players must report for drafts in full equipment. Players having to miss the draft
must make record of it to the Arena. Ride requests will be considered, but not
guaranteed and must be in writing by March 15, 2011. NO EXCEPTIONS!
All players - $75.00 due at time of registration. Any credits will be deducted from
final balance. Final Balance is due by April 15, 2011. Family discounts do not apply
for Spring League.
## Players may receive a $25.00 credit off their final balance, for each
new player they enroll, never before registered with USA Hockey (maximum of 3).
Approximate Spring League Time Frame
Drafts: March 16-18
Practices: March 22-31
Games: April 1 - May 20
